Please
arrive 30 minutes early for your scheduled appointment.
You must bring the following items in order for your
exam to be performed:
• Medicare and/or insurance cards
• Picture ID or Driver’s License
• This referral sheet and authorization if
required
• Prior X-rays, if requested.
Continue to take your medications while following the
instructions below.
Please call us with any questions or concerns about
your imaging study.
MRI:
Allow 1 hour for the examination. Please bring any previous
x-rays that are related to this exam. If you have a
pacemaker, electro-mechanical device, aneurysm clip,
or metal devices or fragments in your body, please let
us
know immediately as you may be unable to have the examination.
Also, please let us know if there is a possibility
of pregnancy. Please avoid wearing makeup or jewelry
as these may interfere with the exam.
